Mill Test Reports are the foundation of material compliance in steel supply chains. They certify that the chemical composition and mechanical properties of a material meet the required standard. Yet the verification of these certificates is still performed manually across much of the industry.
Quality teams review hundreds or thousands of numerical values across inconsistent document formats, converting units and comparing results against specification tables. Under these conditions, even experienced engineers can overlook discrepancies.
These errors are rarely discovered immediately. Instead, they often surface months later — during fabrication, regulatory audits, or after installation — when the cost of correction is significantly higher.
This article outlines twelve of the most common mistakes that occur during manual MTR verification and explains why automation is rapidly becoming a necessary component of modern quality control.
The Reality of Manual MTR Review
A single mill certificate can contain dozens to hundreds of individual values, including chemical composition elements, mechanical properties, hardness measurements, heat numbers and traceability identifiers, and additional specification requirements.
Quality engineers must manually interpret document structure, locate relevant tables, convert measurement units, compare each value against specification limits, and confirm compliance with applicable standards.
This process is both time-consuming and cognitively demanding. Even the most diligent reviewers cannot maintain perfect concentration across large volumes of numerical comparisons.
1. Specification Limit Oversight
Values that exceed specification limits by small margins may be overlooked.
Example: A carbon content of 0.31% on an ASTM A106 Grade B certificate. The specification limit is 0.30% max. The difference appears small — one hundredth of a percent — but it still represents a non-conformance. This is one of the most frequent errors in manual review because the human eye naturally rounds and approximates.
When you are comparing hundreds of values in a single review session, a value of 0.31 sitting next to a limit of 0.30 can easily pass unnoticed.
2. Unit Conversion Errors
Mechanical properties may be reported in different unit systems depending on the mill's location and standards.
Example: Yield strength reported in MPa on a certificate from a European or Asian mill, while the specification limit is listed in ksi. 1 ksi = 6.895 MPa. A reported yield strength of 380 MPa is approximately 55.1 ksi — but a reviewer performing mental arithmetic under time pressure might miscalculate and accept or reject material incorrectly.
This error is particularly common in international supply chains where certificates arrive in a mix of metric and imperial units.
3. Hardness Scale Confusion
Hardness values may be expressed in different scales:
- •HRB (Rockwell B) — used for softer materials
- •HRC (Rockwell C) — used for harder materials
- •HBW (Brinell) — common on European certificates
- •HV (Vickers) — used in some specifications
A value of 95 HRB is approximately 20 HRC. A reviewer who sees "95" and compares it against an HRC limit of 22 max might conclude the material is non-compliant — when in fact 95 HRB is well within the acceptable range. The reverse error is more dangerous: reading an HRC value as HRB and accepting material that is significantly harder than permitted.
For sour service applications governed by NACE MR0175/ISO 15156, hardness limits are critical safety parameters. A scale confusion error here can put non-compliant material into corrosive service.
4. Misinterpreting Table Layouts
Mill certificates vary widely in structure. Tables may span multiple pages, combine chemical and mechanical properties in a single table, use unconventional column labels, or present data in formats specific to the issuing mill.
A certificate from a Japanese mill looks nothing like a certificate from a Brazilian mill, which looks nothing like a digitally generated document from a North American producer. When a reviewer encounters an unfamiliar layout, the risk of misreading which value corresponds to which element increases significantly.
This problem compounds with scanned or faxed documents where print quality is degraded, column alignments are skewed, and decimal points may be difficult to distinguish from scanning artifacts.
5. Missing Required Elements
Certain standards require reporting of specific elements that are not always present on every certificate:
- •Boron (B) — required by many pipeline specifications; even trace amounts affect weldability
- •Niobium (Nb) — required for microalloyed grades
- •Vanadium (V) — required for certain strength grades
- •Nitrogen (N) — required when nitrogen-bearing alloy additions are used
If these elements are absent from the certificate, the material may not meet specification requirements — but the absence of a value is harder to catch than an incorrect value. Reviewers focused on verifying the numbers that are present may not notice what is missing.
6. Ignoring Derived Value Requirements
Some standards require calculated values that may not be reported directly on the certificate:
- •Carbon Equivalent (CE) using the IIW formula: CE = C + Mn/6 + (Cr+Mo+V)/5 + (Ni+Cu)/15
- •Pcm (critical metal parameter) for low-carbon grades: Pcm = C + Si/30 + (Mn+Cu+Cr)/20 + Ni/60 + Mo/15 + V/10 + 5B
For API 5L PSL2, if carbon exceeds 0.12%, the CE value must not exceed 0.43. If carbon is 0.12% or below, the Pcm must not exceed 0.25. Many certificates report the individual elements but omit the calculated CE or Pcm. A reviewer who checks each element individually may find them all within limits — while the derived CE value exceeds the maximum.
7. Product Analysis vs. Heat Analysis Confusion
Certain standards require both heat analysis and product analysis:
- •Heat analysis is performed on the molten steel at the ladle
- •Product analysis is performed on a sample taken from the finished product
The tolerance limits differ. Product analysis limits are typically wider than heat analysis limits to account for segregation in the solidified steel. For API 5L PSL2, both analyses are mandatory.
A reviewer may verify the heat analysis and assume the job is done — overlooking the fact that product analysis is also required. If the certificate reports only heat analysis for a PSL2 order, that is a non-conformance even if all the heat analysis values are within limits.
8. Overlooking Additional Standard Requirements
Specifications often contain requirements beyond basic chemistry and tensile testing:
- •Charpy impact testing at specified temperatures
- •Hardness limits per NACE MR0175 for sour service
- •Supplementary requirements (e.g., API 5L SR6 for fracture toughness)
- •Hydrostatic testing with specific pressure and duration requirements
These requirements may be documented in different sections of the certificate or in supplementary test reports attached as separate pages. A reviewer focused on the main chemistry and mechanical tables may not check whether the impact test temperature matches the specification requirement, or whether the supplementary requirements listed in the purchase order have been satisfied.
9. Language Interpretation Errors
International supply chains frequently produce certificates in the language of the issuing country. A certificate from a Korean mill may use Korean terminology for test descriptions and certification statements, with numerical values in a familiar format but labels that require translation.
Even when certificates include English translations, terminology differences can cause misinterpretation. "Streckgrenze" (German for yield strength), "limite d'elasticite" (French), and "limite de fluencia" (Spanish) all refer to the same property — but a reviewer unfamiliar with these terms may not map them correctly to the specification requirements.
10. Copy-Paste Verification Practices
Some manual review processes rely on copying values from the certificate into a spreadsheet for comparison against specification limits. This introduces additional opportunities for error:
- •Transcription mistakes when typing values from a scanned document
- •Copying a value from the wrong row or column
- •Formula errors in the comparison spreadsheet
- •Version control issues when multiple reviewers use different spreadsheet templates
Each manual transcription step is a potential failure point. A study of data entry accuracy in healthcare (a similarly high-stakes field with numerical data) found error rates of 1–5% in manual transcription tasks — a rate that is unacceptable when every single value must be verified correctly.
11. Review Fatigue
Human concentration declines during repetitive tasks. Cognitive research consistently shows that sustained attention to detail degrades after approximately 20–30 minutes of continuous numerical comparison.
A quality engineer reviewing ten certificates in sequence will be sharper on certificate one than certificate ten. The errors that occur at the end of a review session — the marginal non-conformances, the missing elements, the unit mismatches — are precisely the errors that automated systems catch consistently.
This is not a criticism of individual competence. It is a fundamental limitation of human cognitive architecture when applied to repetitive, high-volume numerical verification.
12. Incomplete Audit Trails
Manual reviews often lack a detailed record of which values were checked, which specification limits were applied, and why approval decisions were made.
When an auditor asks "who reviewed this certificate, what limits did they apply, and did they verify the carbon equivalent?" — the answer is often a signature on the face of the certificate and the reviewer's recollection. This creates challenges during ISO 9001 surveillance audits, API Q1 quality system audits, regulatory investigations, and root cause analysis after non-conformances are discovered in the field.
A complete audit trail is not just good practice — it is a requirement under most quality management systems. Manual processes make this requirement difficult to satisfy consistently.
Why These Errors Matter
The consequences of incorrect MTR verification compound downstream:
- •Non-conforming material enters fabrication — rework costs, schedule delays, and potential structural compromise
- •Regulatory violations — fines, project shutdowns, and loss of certifications

The later the issue is discovered, the greater the financial and safety impact. A non-conformance caught at receiving inspection costs a rejection and replacement shipment. The same non-conformance discovered after the pipe is welded into a pipeline costs excavation, cut-out, re-welding, re-inspection, and project delay.

Modern supply chains are processing far more certificates than in previous decades. Large pipeline projects may involve thousands of heats from multiple international suppliers, each with their own certificate formats, languages, and documentation practices.
A single offshore project might require verification of certificates against API 5L, API 5CT, ASTM A694, ASME SA-182, and NORSOK M-630 — each with different chemistry limits, mechanical requirements, and supplementary conditions. The combinatorial complexity of verifying every value against every applicable requirement across thousands of documents exceeds what manual processes can reliably deliver.
